ServiceLink of Grafton County is part of the Aging and Disability Resource Center network located across the state and may be reached toll free at 1-866-634-9412. Whether you’re trying to untangle the directions for a Medicaid application or trying to figure out which supplemental insurance will be the best match for your Medicare plan, ServiceLink of Grafton has experts who are well versed in all the above issues and much more. They provide an invaluable service for helping individuals and their family members find resources to help them maintain independence and stay healthy.
Our certified staff offers Medicare counseling, caregiver counseling, long-term care options counseling and general information, referral and assistance for services in Grafton County. The Centers are free, accessible and open to the public. Through counseling and education, we help people make informed decisions about obtaining and managing care options that best meet their needs whether through public or private resources. A publicly available search database is found at nh.gov/servicelink. Powerful Tools for Caregivers classes and caregiver support groups are offered throughout the county. Call or go to nhcaregiverconnections.org for class and event schedules, as well as volunteer training and management resources.
Assistive technology device demonstrations, loans and reuse activities are being supplemented by the ATinNH program.
