Our Mission
The Purpose of Grafton County Senior Citizens Council is to develop, strengthen and provide programs and services which support the health, dignity and independence of older adults and adults with disabilities living in our communities.
Consumer Privacy Protection Policy
As participants in programs offered by Grafton County Senior Citizens Council, Inc. and as a consumer of health and human services, consumers have the right to control all personal information gathered in the process of providing those services.
Information we collect:
Information we receive from consumers during the application for services.
Information about services received by consumers from GCSCC.
Information we receive from family members, physician, health care facility.
Unless it is specifically stated otherwise, no other information will be collected from consumers.
Information we may disclose to third parties:
Information about the services consumers have received from GCSCC to NH Dept. Health & Human Services for the purpose of billing for services provided.
Information essential to obtaining services consumers have requested and which are provided by a third party, but only after having received a signed authorization.
Information to provide assistance to consumers in emergency situations.
We do not disclose any nonpublic personal information about GCSCC participants or former participants to anyone except as permitted by law.
GCSCC practices regarding information confidentiality and security:
We restrict access to nonpublic personal information about consumers to those employees who need to know information in order to provide services.
We maintain physical, electronic and procedural safeguards that comply with federal regulations to guard nonpublic personal information.
Reservations of the right to disclose information in unforeseen circumstances: In the event that another service provider assumes the responsibilities of GCSCC for service provision to older and disabled adults residing in Grafton County, we reserve the right to transfer information (including but not limited to an address, name, age, gender, zip code and other information that consumers provide through communication with GCSCC staff) to a third party entity that: 1.) provides similar community based services to older and disabled residents of the area; 2.) agrees to be GCSCC’s successor with regard to maintaining and protecting the information collected; 3.) agrees to the obligations of this privacy statement.
Consumers have the right:
To an explanation of how personal information may be used or disclosed
To control the release of personal information
To consent, in advance, prior to any release of health information
To see a copy of health records
To request a copy of health records
To request an amendment of health records
To obtain documentation of any disclosure of personal health information
Grafton County Senior Citizens Council, Inc. is mandated to comply with these requirements and:
To safeguard consumer personal information by keeping that information in locked files
To insure that computer files containing client records are password protected.
To establish and enforce policies restricting access to consumer records
To require written authorization from consumer prior to releasing any information, excepting in the case of medical emergency
To respond promptly to consumer requests for access to records
To amend information in consumer files in response to requests
To provide documentation of any disclosure of consumer personal health information
To comply with all other requirements of the Health Insurance Portability and Accountability Act of 1996 (HIPAA)
