Our Mission
The Purpose of Grafton County Senior Citizens Council is to develop, strengthen and provide programs and services which support the health, dignity and independence of older adults and adults with disabilities living in our communities.
Home Care Services
At certain times in our life, we all need a little help. Grafton County Senior Citizens Council can assist you with services through a variety of different options.
"Homemaker Services" provides services under both Choices for Independence and Title XX. Clients receiving "Choices for Independence" are referred from case management agencies and are usually nursing home eligible clients who have chosen to stay at home with services in place. Title XX homemaker clients are individuals who have beeon qualified for services by the program coordinator. The eligibilty requirements for this program include, being a New Hamshire resident, age 18 or older, elderly and/or an incapacitated individual. The individual's maximum income can be no more than $1116.00 a month (including interest and dividend income). Resources are not included in the eligibility decision.
"Elder Care in the Home" provides services on a sliding fee scale based upon a person's annual income. To be eligible for this program an individual must reside within the Upper Valley or the Mascoma Valley and be either 60 years of age or older, or over the age of 18 and in need of services due to a chronic illness or handicapping condition.
"Adult In-Home Care" is a service provision program in which the Bureau of Elderly and Adult Services qualifies the applicant to receive in home services. The eligibility requirements for this program include being a New Hampshire resident, age 18 or older, elderly and/or an incapacitated individual. The individual's maximum income can be no more than $1116.00 a month (including interest and dividend income). Resources are not included in the eligibility decision.
All of these progams provide most of the following:
Companionship
Help with light housekeeping, including laundry, dusting, vacuuming and mopping, meal preparation and washing dishes.
Assistance with chores, including budgeting and bill paying, writing letters and doing errands such as shopping.
